Did India have the world's largest outbreak of poliomyelitis associated with injections of adjuvanted DPT?

چکیده
The recent report of an explosive outbreak of poliomyelitis in an orphanage in Delhi(l) and the subsequent correspondence(2,3) have highlighted the phenomenon of 'provocation poliomyelitis'. If an intramuscular injection is given during silent poliovirus infection, paralysis may set in within a day or two. In a community in which poliovirus circulation is intensive, children are at increased risk of developing paralysis for one month after the injection; only those children who happen to get infected during that interval are at risk. The more intensive the endemicity of polioviruses, the greater the probability of provocation poliomyelitis.

S utter et al. (1991) also wrote: "Among the most disturbing features of the [paralytic poliomyelitis] outbreak [in Oman] was that it occurred in the face of a model immunisation programme and that widespread transmission had occurred in a sparsely populated, predominantly rural setting." This represents further evidence that vaccination caused the outbreak. The vaccinators had travelled into s...

Since 1914 there have been many reports of explosive outbreaks of poliomyelitis in orphanages or hospitals, all of which have been associated with injections of inflammatory substances. The earliest in Berlin in 1913 had a paralytic attack rate of 23% and further epidemics were reported from Rome and Naples until the 1950's (see reference 1 for a review of these and other epidemics). In these e...

متن کاملIntramuscular injection as a provocative factor in paralytic poliomyelitis.
Acute paralytic poliomyelitis is still prevalent inspite of large scale immunization programmes being carried out throughout the country. One of the important factors incriminated to provoke paralysis is administration of intramuscular (IM) injections especially, when the child has had exposure to poliovirus(l). IM injections are commonly prescribed in cases of fever(2). The relationship of par...
